Reporting of Major Animal Welfare Incidents
The Canadian Council on Animal Care (CCAC) would like to remind all certified institutions to notify the CCAC regarding any major animal welfare incidents within 10 days of their occurrence. This can be done either by completing and sending in the Major Animal Welfare Incident Self-Reporting Form, or informing the CCAC of the incident by phone or email, and sending in the form shortly after.
This form should be signed by the animal care committee chair and a copy should also be forwarded to the senior administrator responsible for the ethical animal care and use program at the institution.
